The Medical Director leads the interdisciplinary team in every patient’s plan of care and caters it to meet their needs.
After developing a personalized plan of care, this plan of care will be communicated to others of the hospice team and the patient’s caregivers.
In addition, the Medical Director will modify the plan of care and notify everyone of the changes. They are a key resource that the team and the patient’s family call to in situations where matters arise about the plan of care or the patient’s condition.
